Application Guidelines
The Foundation has three grant cycles per year:
Application Deadline / Decision Date
March 15 (spring) / June grants
August 15 (fall) / November grants
December 15 (winter) / March grants
We only accept online applications through this website. No supplementary paper materials are accepted.
If your organization is interested in applying for funding, please read our mission statement and Instructions below.

Our upcoming deadline is December 15, 2008. The online application will be accessible from November 11 through December 15.

Step 1:
Registration
If you do not have an account, you need to register first. If you have an account already, you may sign in with your User ID and Password.
Contact information and Organization Contact
All applications must include:
1) Executive Director's name, address, telephone and email
2) name and title of person completing the application
3) organization's website
Please note: All contact information is required. For grant writers and consultants, please include the Executive Director's information in the Organization Contact section.
Step 2: Questions and Upload PDF Documents
Step 3: Click Submit your Application
The online application will close
at midnight on December 15, 2008. Please do not wait until the last minute to submit your application.

Application Questions
Please note: the information below are the questions you will find in the online application. This is not the online application.
Q1. Project Name
Enter project name or purpose of your application. If the organization is applying for General Operating Support, enter General Operating Support.
Q2. Category
Choose category that best represents the project or program for which funding is requested.

List the borough(s) that the organization serves.

Q5. Grant Purpose Summary
Provide a one sentence summary of the project or program for which funding is requested. If the organization is applying for General Operating Support, enter General Operating Support.
Q6. Amount Requested
Provide the dollar amount requested.
Please note: The foundation’s grants range from $10,000 - $25,000.
Q7. Program Budget Amount
If the organization is applying for Program Support, Capital Support or Other, indicate the dollar amount of the program budget for the current fiscal year.
Q8. Organization Budget Amount
Indicate the dollar amount of the organization's budget for the current fiscal year.
Q9. Cover Letter
Provide a brief introduction of the organization including the Executive Director's name and contact information. (2,000 characters maximum).
Please note: The character limit count includes spaces. It is recommended to use MS Word count (under tools) before copying and pasting the text to the online application.
Q10. Mission and History
Provide a brief description of the organization's mission and history. (4,000 characters maximum).

Q11. Proposal
Provide a brief proposal which outlines the project or program, for which the funds are sought. This should include a detailed account of how the funds will be utilized, a history of the program, and its goals and objectives. (4,000 characters maximum).

Q12. Board of Directors
Provide a list of the organization's Board of Directors. Upload a PDF file (2MB file maximum). See How to save PDF files section below.
Q13. 501(c)3
Upload the organization's 501(c)3 determination letter as a pdf file (2MB file maximum). See How to save PDF files section below.
Please note: the Foundation cannot accept applications from organizations with a fiscal sponsor or whose tax-exempt status is pending. Organizations applying for a grant must have received an IRS determination letter.
Q14. Program Budget Description
If the organization is applying for Program Support, Capital Support or Other, provide an annual program budget for the current fiscal year.

Q15. Organization Budget Description
Provide an annual organizational budget for the current fiscal year. Upload a PDF file (3MB file maximum). See How to save PDF files section below.
Q16. Audited Financial Statement
Upload the organization's most recent audited financial statement as a PDF file (5MB file maximum). See How to save PDF files section below.
Q17. Current Funders
Provide a list of all funding sources (corporations, government, individuals, board of directors, grants, etc.) for the current fiscal year. The list should include names and amounts contributed and projected. If the organization is applying for Program Support, Capital Support or Other, please include funding sources for the organization and for the specific program. Upload a PDF file (2MB file maximum). Q18. History with LAF
If the organization has received funding in the past, provide the funding year(s) and grant amount(s). (example: 2007: $10,000; 2005: $5,000; 2003: $15,000).
